ADULT SOCIAL SERVICES
The Physical and Sensory Impairment Team provides care management and re-enablement support to adults with physical disabilities; long-term conditions; HIV/AIDS; asylum seekers and sensory impairments and carers. Many of our service users have needs which are several and complex.
The team is committed to outcome focused care and ensuring that service users are able to reach their maximum level of independence.
Several new opportunities have arisen in this exciting and innovative team which is making positive changes to the lives of our service users and their carers.
Re-enablement Team
A qualified social worker, you will have experience of working with service users with physical and/or sensory impairments and their carers. You will also have expertise in working in complex situations where there are often several needs.
You will have in-depth knowledge of relevant legislation, be able to work under pressure and be able to plan and prioritise your own workload. With an awareness of relevant performance indicators you will be able to complete reviews within relevant timescales.
You will need to demonstrate the ability to work in partnership with service users and their carers to adjust and devise appropriate support plans focused on outcomes and to maximise independence.
The ability to travel around the borough is essential.
