Main Purpose Of Job
The team manager has full responsibility for the effective day-to-day team management of approximately 20 staff employed by Medway Council and will be expected to oversee Medway Community Healthcare staff in the absence of the Deputy Locality team manager.
To be responsible for the allocation of team resources and financial management of allocated budgets.
The skill mix in the team includes Social Workers, Care Managers, Community Nurses, Occupational Therapists and Physiotherapists.
The Team Manager must ensure the team complies with all statutory requirements, including Medway Council’s responsibilities as the lead agency for the protection of vulnerable adults. This post will also have an overview of the Self Directed Support work in Medway.
The Integrated Team provides assessment, care planning, purchasing and provision of health and social care services to a defined population and within set criteria.
